i use stickam.com
but there are plenty if you look hard

while i have not done it this way myself, people subscribe to dynamic DNS or dyndns and get a web address like joeschmeau dot dyndns dot org or something and it updates to follow your changing IP and people can browse to your web server. im not sure how well this works with video streaming products though..
